Definition

The discussion text in Indonesian is a text that contains two different opinions regarding a particular issue or problem that has become a controversy in the community. The discussion text has linguistic characteristics, including using

Pros and Cons of MR Immunization*

MR immunization is an MR vaccine that is given to prevent the occurrence of diseases caused by the Meales (measles) and rubella (German measles) viruses. Measles is caused by an RNA virus. RNA viruses are viruses whose genetic material is in the form of nucleic acids in the form of twisted double chains and belongs to the Paromyxavirus family. This virus spreads through the air so it can easily attack people who have weak immune systems, have never been exposed to measles, and have never had the measles vaccine. Then this virus will spread throughout the blood vessels and infect the body 2-3 days after the body is exposed to the virus.

Like measles, rubella is also caused by an RNA virus. The difference is, the virus that causes rubella is a single-stranded virus that belongs to the Rubivirus genus. This type of virus generally attacks children aged 3-10 years through the air and is transmitted by the mother to the fetus. When the virus infects the body, the virus will enter the body's cells and stay there for a while. When the immune system weakens, the virus will reactivate. If the virus strikes a girl and she doesn't realize she has the rubella virus until adulthood, automatic she will transmit the virus to her fetus when she becomes pregnant later. The baby she will be carrying will be born with congenital rubella syndrome (congenital rubella syndrome). Therefore, prevention must be done early by giving the rubella vaccine.

Until now, there is no drug that effective treat measles and rubella. Therefore, what we can do is to do prevention as early as possible, namely by giving the MR vaccine which is proven to be very effective in preventing measles and rubella. Since the discovery of the MR vaccine in the 60s until now, more than 141 countries have used measles and rubella vaccines.

How about Indonesia?

Indonesia itself is committed to achieving measles elimination and controlling rubella or congenital rubella syndrome (CRS) by 2020. The target of MR immunization is all children aged 9 months to 15 years throughout Indonesia. The MR immunization was given regardless of immunization status or previous history of measles and rubella. The implementation of the MR mass immunization is divided into two phases, namely the first phase in 2017 carried out throughout Java and the second phase in 2018 for areas outside Java.

According to the Indonesian Ministry of Health, the MR vaccine used in Indonesia has received a recommendation from the World Health Organization (WHO) and a distribution permit from the POM. So, MR vaccination is safe to do. Why does Indonesia need to carry out MR immunization? Data from the Ministry of Health of the Republic of Indonesia recorded that from January to July 2017, there were 8099 suspected rubella cases (2535 positive for measles and 1549 positive for rubella). When compared with case reports after the implementation of mass immunization on the island of Java, the report cases decreased to 1045 suspected rubella measles (38 positive for measles and 176 positive). rubella). During the last four years until July 2018 data from hospitals throughout Indonesia recorded 1660 cases of CRS had occurred. However, the government's efforts to eliminate and prevent measles and rubella by administering the MR vaccine are not easy. Although the MUI recently stated that it is "allowed" to administer the MR vaccine, the fact is that there are still many people who refuse to give the MR vaccine.

MUI stated that the MR vaccine is not halal because it uses pork enzymes in the manufacturing process. However, this vaccine can still be given because there are several conditions. For example, the impact of the dangers if no vaccine is given and there are no other halal vaccine alternatives in the world that can be given. Rejection of the MR vaccine is widely practiced in various regions. In Riau, until almost one month after the implementation of MR immunization, only 14 percent of children have received MR immunization. The Riau Provincial Health Office revealed that the low coverage of MR immunization in Riau is inseparable from the issuance of the MUI fatwa.

The government launched the provision of MR immunization solely to eliminate MR disease in Indonesia. This is due to the high number of MR cases that have occurred worldwide in recent years. However, there are still many people who do not pay attention to this fact. This is further exacerbated by the circulation of hoax news related to MR immunization in the community so that it makes people nervous. Therefore, health workers have a very important role to provide information true to society. This needs to be done to achieve the eradication of measles and rubella by 2020.
