Definition of Rubella
The definition of rubella or another term for German measles is a disease caused by a virus. Rubella usually affects children and adolescents. Based on data from WHO, in 2016 Indonesia had 800 cases of rubella confirmed by laboratory tests.
Most people think that rubella is a mild disease, but don't underestimate this disease if it occurs in women who are pregnant. Because it will cause serious effects for the fetus in the womb. Rubella or German Measles generally occurs in children, but since a vaccination program was carried out, this disease has actually been eradicated even though there are still children who are infected too.
If anyone is attacked by this disease, then you should quickly report to the Health Service because this disease must be destroyed. In a sense, each doctor who diagnoses this infection must by law inform the Health Service. This is of course to identify the source of Rubella infection and quickly eradicate it.
Symptoms of People Affected by Rubella
Symptoms of rubella virus disease are the presence of a specific red rash, at first the spots that cause it itching, starting from behind the ear then going to the head then to the neck and spreading to all parts of the body on.
This rash usually lasts for one week. Another symptom that is often experienced is swelling of the lymph nodes behind the head and the ear area, body temperature increases to 38 degrees Celsius and even more followed by the body chills. Symptoms that cannot be recognized in adults are inflammation and joint pain or arthritis and arthralgia.
In some rare cases, this disease can cause serious complications. 1 in 6000 cases, rubella can cause inflammation of the brain. 1 in 3000 cases can affect blood clotting. The incubation period for rubella is 14-21 days, most of those who develop a rash are between 14-17 days after exposure.
Rubella Treatment
Treatment or handling of these cases is usually mild and does not necessarily require special treatment for this condition. Often the symptoms will subside within 7-10 days and the rash will go away on its own.
Medicines that can be consumed to reduce fever and make aches and pains mild can take paracetamol and ibuprofen according to age and according to doctor's recommendations. Patients are also advised to drink lots of water.
Rubella prevention
A good way to provide prevention of rubella is to have immunizations with measles, mumps and also rubella or MMR. The MMR vaccine can be given to children as part of a routine vaccination program.
While the first dose can be given around 12-13 months and the second, before children start school, generally between the ages of 3 and 5 years.
Women who want to have plans to become pregnant can ask their doctor to do an immunity test for rubella. If you have little immunity, you can do the MMR vaccine and this vaccine can be given at any time up to one month before getting pregnant.
Breastfeeding mothers are safe to get the MMR vaccine. If you feel that the immunization that has been carried out is not enough, you can request the MMR vaccine at any time.
Rubella that occurs in pregnant women
Some women who already have immunity to rubella, have either been infected as a child or from vaccinations. But for women who are not yet immune to this virus, they can transmit it to their unborn fetus.
Rubella can cause birth defects as Congenital Rubella Syndrome or CRS. These defects can be mental defects, deafness, cataracts, abnormal heart and also slow or abnormal fetal growth.
The risk of a baby getting this syndrome and the severity of the birth defects that will be experienced depend on the gestational age when the mother is infected with rubella, namely:
- Mothers are infected when the gestational age goes to 11 weeks, the risk is 90%.
- Mothers are infected when the gestational age is between weeks 11-16, the risk is 10% -20%.
- Mothers are infected when the gestational age is between 16-20 weeks, so the minimal risk is experiencing deafness.
- Mothers are infected when the gestational age is between the 20th week so there is no increased risk of this syndrome.
Women who are pregnant should consult their doctor if they are exposed to rubella or if they have been with someone who is infected with rubella for 15 minutes. If you haven't gotten the vaccine, tests can be carried out with immunization and you may be referred to a specialist obstetrical examination to be carried out intensively so that it is checked whether the baby has a syndrome or No.
Performing an ultrasound and amniocentesis can determine the type and extent of the birth defect. The treating doctor can offer counseling to pregnant women, so that the mother can make the next decision, whether she will continue the pregnancy with the risk of defects that have been confirmed.
